This article provides information about Replacing a Health Certificate for Dairy REX.
Overview
A completed certificate OR a REX with the status of "COMP" can be replaced if the information contained in the certificate requires amendment. You will need to submit a request to the department for review. Kindly follow the below steps to submit a replacement certificate:
1. Give the reason for Replacement under the "Replace Certificate Reason" field.
2. Kindly update the REX and save.
3. Select messages type to "Replace" and submit the REX.
4. Read the declaration.
5. If you agree, select ‘Submit’.
The request will go to the department for action. You will receive a message in the NEXDOC inbox to advise if the request has been approved or rejected.
